If you have an urgent medical problem that can not wait until normal business hours please call our answering service at (888) 877-1817. If you do not hear back from the doctor or nurse practitioner within 30 minutes please call the answering service again.

Sending Your Child to the Doctor Without You: If your child is under 18 years old and you plan on sending them to their appointment without a legal guardian we must have a signed consent form. You can find the form under the Patient Forms page. We will not be able to provide medical care, including vaccinations, medications or medical treatment without it.
