Are you playing High School sports but are not due for your annual exam? We are offering sports physicals for a flat fee of $35. Your insurance will not be billed. Payment will be due at time of visit. This is a limited exam and as such specific health concerns will not be able to be addressed at this visit. Also, vaccines as well as routine vision and urine screens will not be done. Please bring your forms already filled out and they will be completed by the physician at the time of visit. Please
